#file-notice new hay analysis #file-notice
I've added new Equi-Analytical wet chem #603 analyses for Inky, along with some details of changes in his management to his CH. I have also requested his diet to be mineral balanced. I'm eager to further improve his management. I plan to repeat labs for insulin, glucose and ACTH next week. He has not been in work since August due to lameness, and is just now being ridden at the walk 3X/week for 15-20 minutes. Equioxx was begun in late September and I hope to progress him to trot, then canter before tapering the Equioxx, unless there is a suggestion for an alternative. I recently held his flax because after starting it, he began to have soft, loose stools.

Patty,
Why is he getting Equioxx? -- Eleanor in PA www.drkellon.com |

Dr. Kellon, thank you for asking!
He became lame 8/1/22 after stomping off a chunk of his RF. Had vet do lameness exam and xrays after 2 wks rest with no improvement. She found 3 degrees rotation on RF, known bony proliferation (ring bone and club foot) RF, with coffin and fetlock effusion on exam. (His laminitis event in 2017 was primarily his LF with 3 degrees of rotation.) He also had soreness/decreased flexion in his hind stifles, with joint effusion bilaterally. and back sensitivity lumbar through SI. I wanted to focus on the RF and chose not to do blocking for further eval at that time, but continued rest. Had vet/spine/chiro eval (boards at our barn) for second opinion 3 weeks later who corroborated the lameness and chronic changes and mild arthritis. Both vets suggested a trial of Equioxx, so I started it late September, one pill daily. I continued hand-walking him 3x/week and progressed to riding him at the walk 3x/week. I've been wondering if what he's experiencing is an exacerbation of chronic or subclinical laminitis. In August he also had goopy eyes, hives with insect hypersensitivity, sheath swelling, and dermatitis. I (probably stupidly) followed the advice of the vet/spine/chiro, and changed his Prascend from 1.5 mg daily to alternating 1mg/day with 1.5 mg/day, when I probably should have been increasing it! (She's also the one who insists that re-checking his ACTH at this time is contraindicated d/t 'seasonal rise') Also, due to his lameness, he is no longer in training and getting the exercise which is critical to his PPID management. I do plan to have labs (ACTH, glucose, insulin) drawn 11/8/22 when the vet comes to do vaccinations. Thank you again for your help. -- Patty and Inky https://ecir.groups.io/g/CaseHistory/files/PATTY%20AND%20INKY https://ecir.groups.io/g/CaseHistory/album?id=268362 horse located in SE Wisconsin owner located in north suburb of Chicago joined Aug 2019 |

I forgot to mention the Vet/Spine/Chiro did a session of acupuncture and massage at her initial visit 9/14/22, then a subsequent acupuncture session 2 weeks later. At his most recent re-check 10/20/22 she felt he had improved but did not suggest changing him from riding at the walk 3x/week.

Definitely get him back on his regular pergolide dosing schedule.
Instead of indefinite Equioxx, which even the package insert clearly states is not safe, consider trying Phyto-Quench pellets https://www.uckele.com/phyto-quench-pellet.html . Eleanor in PA www.drkellon.com |

Will do absolutely, thank you

sorry to bug you - you would recommend phyto-quench vs straight devil's claw to replace the Equioxx?

PQ is more effective IME but could start with plain DC; just avoid Yucca.

Thank you - will try PQ
