Hi, Erica.
On restarting pergolide, I’d still restart a bit slowly. Start with half a pill. Give it a couple of days and then increase. If Windy stops eating well or seems distracted and walks away while eating, that’s the pergolide veil. Because not eating is a serious problem, if that happens, immediately reduce the amount of pergolide as described in the link I gave.

You didn't mention if Windy experienced the pergolide veil when you started Prascend. Not knowing that, I would gradually titrate Windy's Prascend dose back up using the usual protocol suggested here: ¼ Prascend for several days, then ½ Prascend for several days, then ¾ Prascend for several days, finally 1 full Prascend.

You didn't mention if Windy experienced the pergolide veil when you started Prascend. Not knowing that, I would gradually titrate Windy's Prascend dose back up using the usual protocol suggested here: ¼ Prascend for several days, then ½ Prascend for several days, then ¾ Prascend for several days, finally 1 full Prascend.

Dear Dr Kellon,

 

In addition I just went on a easy walk trail ride and Windy was tired even going up small hills.  It seemed that the one time she didn’t need to rest was just about the same time I started taking her off the Prascend.  She also wanted to eat the dirt which she hasn’t wanted to do in a long time.

Windy was off Prascend for the past 3 weeks.  Her condition has slowly declined so that by yesterday when I rode her she seemed lame, very stiff and short strided.   Today she has very bad diarrhea and pulses in the hind feet.  Vet came yesterday for blood test (She was very annoyed at me for taking her off the Prascend) Could 3 weeks off the medication affected her so?

Yesterday when I rode Windy up a steep hill she did not need to stop.  Usually she stops 2 or 3 times and is tired at the top. 

Do I continue with the ALCAR twice a day with Karo after exercise indefinitely?  Is there any thing else I can do to continue her improving?

Thank you for the input.  She is off the Prascend  staring tomorrow (1/23/23) and my vet is coming in 3 weeks.  I will upload blood results then. 

 

She has been more supple and not tiring a bit less up steep hills (> 25 % grades) since using ALCAR and dosing with Caro after exercise.  Her hind end muscles are like a rump roast all the time.  I live in an area with some pretty steep grades.   Thankfully  this winter has been mild and daily exercise has been possible with long warm up and cool downs.

 

I’ve owned horses for over 50 years and there is something definitely wrong with the abilty of the hills with this mare. I questioned my vet over the last 10 years when I got this horse without any answers.

 

I thank you and everyone for this group.  You are dedicated and concerned and I appreciate it much.

Pergolide doesn't cause musculoskeletal issues in the experience of thousands here but I agree she shouldn't be on it if she doesn't need it. I'm not convinced that she doesn't, hence the suggested to take her off and retest.

Hi Erica

You mentioned that lately she's developed these symptoms. How long is lately? Did she have these issues during the rise? When did they start?

She looks well controlled on 1 mg and test results suggest she likely benefits from some treatment as per Dr. K. 

I will take the middle road, per my experience with my mare, and suggest that you might try giving her just half a pill and see if it helps?

I think half could be enough to keep her safe and might help alleviate some musculoskeletal issues.

I will be updating my CH ASAP after I get my new hay analysis.  Also I’ll attempt to upload pictures and hoof xrays. When I contact Dr Gustafson on the balancing is that private messaging or through the Forum.  All of my messages in email should have my signature through the Forum?  I thank you and everyone for your patience

 

RE:  ALCAR -  I put Windy on ALCAR twice a day for 2 weeks and her muscles were less tight.  Like a rump roast.  Still tires when going up hills but some improvement.

After two weeks on ALCAR I went to once a day.  Her muscles got tight again.  Went back to twice a day and muscles like a rump roast again.  Much improvement going up hills and not tiring.  Can I keep her on ALCAR twice a day. 

 

BTW I give the Karo after exercise.  Is there another product I can use that is not so “messy”?  Sugar cubes or treats?

Windy's response to TRH was so high I suspect she would test positive any time but the question is what does "positive" mean. Does it mean she has hypertrophy but is not putting out abnormal hormone levels or does it mean she is putting out abnormal levels just not enough ACTH to test abnormal at baseline yet. We don't know. Either way it's probably a good idea to give the Prascend to control it.

With as low as her insulin was before you began Prascend I doubt there's much risk of laminitis if you stop it for a while. You can do a quick wean of half dose for a week then off, retest 3 weeks after off.

Dear Martha, 

 

I don’t understand how to reply to emails.  I’m not sure if I reply to group or sender.  I barely know how to post.  I don’t do this often so it is very confusing. 

 

I tried to search retesting after seasonal rise but I have trouble with searching the files too.

 

If Windy needs Prascend and I take her off won’t she have more of a chance of Laminitis.  Why would I risk it?

How do I take her off the medication? I assume I wean her off.

After I wean her off do I wait 3 weeks after the last pill?

Hi Erica,

Windy's only positive ACTH test was via TRH Stim in a time frame (August of 2021) when that test is not reliable.  Her baseline ACTH for that test was within expected range.  The only way to truly determine if she's PPID and in need of medication would be to stop the Prascend for 3 weeks to make sure it's all out of her system and then repeat the TRH Stim test.  

If you are going to have the vet out for spring shots/Coggins in February or March I'd plan on stopping the Prascend for 3 weeks before that appointment and have them redo the TRH Stim test so you have an accurate result for it.
