Would you please explain the difference (use and effect) of acetyl L carnitine and L carnitine (alcar?)?

I believe L-Carnitine is recommended for Invokana horses where triglycerides are an issue. Your vet needs to be on board with Dr K.
Acytel_L-Carntine is most often used here for neuropathic pain in winter laminitis and general neuropathy.
Dr Kellon, please correct me if I don't have this right.

ALCAR (acetyl-l-carnitine is a metabolite of L-carnitine. When you feed either one the body will do the interconversions to some extent but if you feed L-carnitine you will end up with more L-carnitine and vice versa.
L-carnitine is fed to improve the metabolism of fats.
ALCAR is effective in relieving neuropathic pain in laminitis, helps with nerve regeneration, improves mitochondrial numbers and encourages glucose into energy burning pathways. In humans, it works with metformin in alleviating IR in women with PCOS https://pubmed.ncbi.nlm.nih.gov/34047916/ . In this study, L-carnitine also improved metabolics in IR individuals and increased ALCAR https://pubmed.ncbi.nlm.nih.gov/31676389/ .This study using more advanced type 2 diabetes patients found no response https://pubmed.ncbi.nlm.nih.gov/29696241/ .
Either one certainly won't hurt with EMS but the major application here is neuropathic hoof pain.
