As for the horse that has not improved, they should check first to make sure he/she is actually eating it and after that check ACTH to see if dosage is correct. Even if the root cause is metal toxicity, removing that won't help because the dead neurons cannot be replaced so they need to stay on pergolide.

Sorry, Dr. Kellon. you are correct. it is not referenced in the article. I have a friend who is wanting information about pergolide and HMT. She is aware I am asking the group about any evidence of HMT being helped by pergolide. Here is part of her post.
"Will be testing another older horse that has PPID (Cushings) and determine if the root cause is HMT. I will be documenting the results of all the horses we are testing. Couple with allergies, Lucy because of her momma and the fact she has to also be a low carb, low sugar diet at a young age, and a couple of others. But even if the PPID cases do not have any HMT this study shows the prescribed Pergolide helps these horses and even if they do have HMT they should not be removed from this medication until toxins are removed and further testing can be done. I just cannot help but wonder how many of these horses have toxic metals that caused the issue to begin with, where the Pergolide only maintains, and not truly helping.
The reason I wonder about this is because the horse with PPID here has not improved any and has been on it for some time now.
