Lab Results and New Case History

I hope I have created the correct link to the new case history file for Hershey. Would someone please review the lab results and let me know if I need to be concerned if Hershey is borderline, or is, IR or not. I will update his diagnosis once that has been reviewd. Thank you!

Hi, Chatycady. 

Hershey is definitely IR with that insulin. He is also not a BCS 5 and is closer to a BCS 7. He’s a very handsome guy but there’s weight to lose. 

Suggestions:
1. Analyze your hay. 
2. Drylot and no grazing without a muzzle
3. Review the BCS photos so you can recalibrate your eye. 
4. If you can arrange a helper to hold a cloth tape, do the measurements to calculate Hershey’s weight by weight calculator and compare that to the weight tape. 5. Keep an eye on Hershey’s PPID symptoms. Lethargy is a tough one to evaluate because it’s pretty general. Hoof tenderness makes horses reluctant to move at liberty, while general lethargy and depression can indicate the listlessness of PPID.

His ACTH last October was not sky high but it bears watching given his age. Don’t rely on hair coat as a symptom: it’s a symptom of advanced PPID, and you want a diagnosis sooner than that. I’m a worrier, so if he were here, I’d recheck ACTH, insulin and glucose soon. My experience was that my mare’s ACTH jumped from 35 in October to 56 the following March, clearly PPID based on the March results. I happened to have a progression of ACTH results showing gradual increases, so I wasn’t shocked by the diagnosis. I had plenty of time to get her PPID under control before the next seasonal rise.

Thank you. The "fat" picture was last year, which I rated at a 6. I will note that I need to reevaluate my "eye" and learn more about BCS. I will update his file to positive for IR. I missed the "weight calculator". Will do!

Ah, thanks for pointing out the dates of the photo. I can’t see a date, just Misc. Images. Maybe add dates to your descriptions? A current photo for a current BCS would help other volunteers avoid my mistake of assuming the photos were current. Wowzer!! Ive been trying to fatten up my Appendix quarter. Hes at a 5 based on that picture!  I will stop pronto and re-evaluate all my ponies. Lots of fat horses in my area!

I'm not sure I understand BCS.  Is your mare's BCS a 5 in the picture (link) you provided?  That's where I would have put it.

Thanks.  That's what I thought but I wasn't sure if her original BCS was an under-estimate or an over-estimate.
