Mouse testing for PPID

Mouse was put on Prascend in 2017 without ever being tested for PPID.  He was started on 1 mg and has been on 1mg since.  Another vet wants to test Mouse for PPID.  Can  he be accurately tested while on Prascend and if not how long does he have to be off of it before testing.  How do you taper Prascend if so?  What is the safest, most accurate test to use to see if he can come off of Prascend.  He has no signs of PPID, is clearly IR.  I started Metformin again on 2/18/23 at 30mg/kg 2x a day.  It is being syringed in.  He had blood drawn on 3/1/23 and insulin is 58.63.  Not low enough but so much better.  I will update my CH when I can figure out how to get into the new site.  It will not accept my current log in name or pw and sent me here?  I also have 9 xrays to upload that were taken on 3/1/23.  Thanks Kimberly

Hi Kimberly,

1 - your membership to the new case history group was approved and your email is the same as the one you use for this group.  If you can't log in try resetting your password using the "I forgot my password" button.

2 - As Dr. Kellon noted last year you will need to wean him off the Prascend: https://ecir.groups.io/g/main/topic/91360312#277884

3 - he will need to be off it completely at least 3 weeks before testing ACTH again so see if he actually needs to be on Prascend

4 - you only need to test basal ACTH while he's on medication.  If you want to see if he is actually early PPID once off the medication you can do the TRH test but there is no acceptable scale at this time for doing that test while a horse is on medication.

thanks Sherry, I will try to log in again.  I wanted to ensure there had not been any changes to the taper directions as I do not want to put him at risk and taking him off make me very nervous, even though I do not think he is PPID.

thanks Sherry, just wanted to ensure nothing had changed with the taper instructions and that this is even safe to consider doing.  I have never thought he was PPID and would love not to have to use the drug but do not want to put  him at risk if I am wrong.
