pracent Charlie
Charlie gets every dag starting 26-02-2023 1/4 pracent to start with because of high levels Insuline and bloodtest 0.1 above the top level this season. to help her ?.
The bloodtest Insuline now is going down ( see case history ) is it nesseserry to give the pracent now to keep giving ? Or was it only to help her in the beginning? She stil gets 2x 3000mg metformine every day. -- Gonnie NL , EU 2022 https://ecir.groups.io/g/CaseHistory/files/Gonnie%20and%20Charlie https://ecir.groups.io/g/CaseHistory/album?id=274645 https://ch.ecirhorse.org/case-history.php?id=90 |

Hi Gonnie,
Charlie's ACTH in February was too high, so she probably needs to be on Prascend to control her ACTH, which is a blood marker for PPID or Cushing's disease. It may also be helping her insulin (along with the Metformin), so she should stay on the Prascend. After 3 weeks on Prascend her ACTH should be rechecked. It should be in the middle, or lower end, of the reference range. -- Kirsten and Shaku (EMS + PPID) and Snickers (EMS) - 2019 Kitimat, BC, Canada ECIR Group Moderator Shaku's Photo Album Snickers' Case History Snickers' Photo Album |

Hi Gonnie,
Prascend isn’t administered according to weight but rather increased until the ACTH falls into the normal range. Prascend is meant to be split in half if desired (as noted by the scoring on the tablet) but dividing it further by splitting the half into quarters may not result in equal amounts. My take from this is that they don’t expect horses to need less than 1/2 Prascend. You can divide it more accurately by dissolving the half Prascend in a small, measured amount of water and dividing that in half. I’ve been having trouble finding things recently and the latest is that I cannot find the February test result Kirsten refers to. I looked through your February blood test results (in Dutch) but it wasn’t apparent to me that there was an ACTH test result there for Charlie. Six is incredibly young to be PPID. I might ask to have another ACTH run if you have a high test result but do it before July when the rise will begin to affect the values. -- Martha in Vermont

Hello Martha
Ok I understand it. I will do the pracent the way you advice it. in the case history on 22-2-23 the blood results of EMS have another page when you scroll down you can see the the bloodtest ACTH and also the reference here in the netherlands now. I will do some bloodtest ACTH than in apri/may if that is better than. Thank you for your reply. -- Gonnie NL , EU 2022 https://ecir.groups.io/g/CaseHistory/files/Gonnie%20and%20Charlie https://ecir.groups.io/g/CaseHistory/album?id=274645 https://ch.ecirhorse.org/case-history.php?id=90 |

Thank you, Gonnie! I don’t know how I missed seeing the ACTH because I definitely recall looking at the signature. Was there anything stressful or exciting happening when the blood was drawn? Did you truck her to the vet? I once had the blood drawn after dental work on my horse and the ACTH was much higher than I expected. Sedation could do the same. At your next blood draw, just be sure to keep these things in mind.

Hi Gonnie,
6 is very young for PPID, I didn't notice that when I replied earlier. I agree with Martha about retesting to confirm the diagnosis. To do that she should not be on Prascend for at least 3 weeks prior to the test. The Prascend makes them lethargic and reduces appetite, but it is temporary. She should perk up after a couple weeks. -- Kirsten and Shaku (EMS + PPID) and Snickers (EMS) - 2019 Kitimat, BC, Canada ECIR Group Moderator Shaku's Photo Album Snickers' Case History Snickers' Photo Album |
