Reading ACTH TRH Results - Request for Dr. Kellon's Opinion

Just wanted to check back, Dr. Kellon, to see if you have any recommendations for what to do for Abbey? There is another vet I can call locally who treats many horses with PPID and IR, however, she is the one who told my last vet that we should switch her to Prascend and add Cypro, so I am hoping to get another opinion. And, I especially love that you and this entire group have so many years experience with this and this is your focus whereas our vets around here seem more focused on sport horses, injury rehab, reproduction and the like. Thanks so much!

Hi Lori, Based on your message regarding symptoms it sounds like she needs a bump on her pergolide dosage or switch her to Prascend as per this message from Dr. Kellon - Re: Reading ACTH TRH Results (groups.io) and this one Re: Reading ACTH TRH Results (groups.io). If you do switch her to Prascend Dr. Kellon suggested retesting her in June to see what her results are then.

Thanks Sherry, I guess I thought there might be more to it once she read the answers I provided to her questions around symptoms. I also saw another person posted (Anne Peer) with a horse that had pretty much the same test results where you said to maybe just give him pergolide during the rise, so I guess I am just very confused as to what is best for Abbey. |

I'd not be using anyone who wanted to use cypro. Here's an article that may help, that you may want to slip to one or both of yours. https://drkhorsesense.wordpress.com/2022/04/04/nothing-new-about-cyproheptadine/ IMO, shedding by itself is not a reliable symptom. Your top line concern could be. Can you get some pictures and post them in your new CH under Weight and BCS? We'd want to see an oblique shot from shoulders to butt to see how her back muscles are. The new site has of examples of BCS, and if you scroll to the bottom you will see what I mean about the oblique topline shot. The horses in the example are overweight. I use this only as example of camera angle. I also wonder about the impact of time of day of the TRH testing -- 9:30 am vs 5:30 pm -- and would there be that large of an impact, if any, as a result. (EDIT: I see Dr Kellon answered this in previous response to you.) If she were here, I would probably have her endogenous ACTH tested in May or so when it is lowest, and then, if in the budget, in late July to see how she is on 2 mg or what ever your current dose is. I'd log symptom review everyother day or so. The new site is perfect for that. You know your horse; go with your gut. Thank you Nancy, That is the original post, thanks for linking that here, I didn't realize it was going to start a new topic when I edited the subject. Thank you also for the article about Cypro, that is good info to pass on if I can find a vet who is willing to listen. I did post a photo of Abbey, it wasn't sideways on my screen, but it saved in the site sideways, hopefully it's still easy to see her topline.
The reason I was wondering about the timing of the test is I give her the pergolide in the morning around 8 or so (although the first time she had the test she wasn't even on it yet), I was just wondering if by 5:30ish it would be partially out of her system and that's why the Post # was higher at that time of day. Well, I'd call that a lateral view, but IMO one can see of loss of top line. Uncontrolled PPID will attack the short muscles in the back, hence the loss of top line.
