Request for everyone using compounded pergolide

I've seen everyone's comments, including about customer service, and that is certainly a valid consideration. However, I would like to ask everyone using compounded pergolide from any pharmacy to ask if they have conducted stability testing and if they would be willing to share that with the group. Obviously if you have good stability testing there would be no reason not to share it!

I would love to help but I do not have a clue as to how to conduct a stability test.
I can only tell you that even tho my last two shipments came in thru the worst heat ever, they worked!  As good if not better than expected.

We use Wedgewood Pharmacy in NJ and can ask them. (Hopefully they won't get too many emails from everyone on this list.)

By the way don't look at our case history Buffalo's is way out of date.  No laminitis, he looks gorgeous and his ACTH was down to 19 last fall so I figured I'd wait until the vet comes out next week to update it.

I can reach out to Avrio and Prescision as I am shopping for new RX for 12mg for Vinnie and 4.5mg for Summer, and Incan ask for stability tests st the same time.

Vinnie has had significant increases f pergolide this year to manage him so I am curious about the stability question in context to the progression of his disease.  

Summer too has had pergolide increases, as her ACTH came back at 49 vs 18 from a few months earlier.  She was moved to an oil suspension because she started giving me trouble with capsules.

I asked Avrio several times for their stability testing when they told me the compounded pergolide was fully effective 180 days and heat/moisture did not affect it. (The pharmacy they purchased - Pet Health Pharmacy out of Youngstown, AZ- told me it was only fully effective for 90 days and to order 100 pills at a time so they would be a fresh batch as we have 4 horses on it.)

We live in a rural area in the hot desert so I don't like it sitting in my metal mail box for long.  I have it shipped overnight or second day.  Phoenix (where the rx is compounded) is about 400 miles or 6 hours from us. So I used to get my pergolide Wednesday if I ordered Monday and they created a new batch. Since they were purchased by Avrio, the service has really gone downhill and the prices uphill. The last shipment went like so: Ordered August 9, arrived August 18 - VIA PUERTO RICO. When I told them it was in PR, they would not ship me another order without a vet rx, would not give a refund, or make any kind of an adjustment.  Nor would they provide their stability testing data. My vet is really unhappy and she doesn't like to prescribe compounded.  So, I'm hoping she will continue and I can find a new ethical compounding pharmacy.

All of you using compounded pergolide should ask for actual copies of their stability testing.  If they don't have it - and if their volume is low that's reasonable - they should say so. The potential amount of business from this group for these pharmacies is huge. We deserve, and should demand, transparency.

I requested info from Thriving Pets, response below. It sounds like they haven’t done their own testing, and are relying on what the manufacturer says, plus good practices to protect the medication.

Hi, Eben. Thank you for contacting us about Pergolide stability. The answer is a bit complicated so I will summarize our thoughts and please feel free to ask any questions you have.

Pergolide stability:
Pergolide is sensitive to light, air, humidity, and heat. This is why ThrivingPets recommends the capsules (protects the drug from air and humidity). The vials we use to dispense the capsules protect the medication from light. We also recommend storing Pergolide in the refrigerator to protect the drug from heat. ThrivingPets is the only compounder that specifies our brand of Pergolide- Teva, and has our supply chain store our Pergolide in the refrigerator. Below are the recommendations from the US Pharmacopeia and ThrivingPets for beyond-use-dates (expiration dates) for compounded Pergolide.
- US Pharmacopeia- 180 days
- ThrivingPets recommendation- 100 days

I checked your order history and it looks like you are typically getting 120 capsules at a time. Reducing the number of capsules you purchase may slightly improve the potency and efficacy of the medication. Changing to 100, 90, or 60 capsules would likely make a very small difference, like 1 to 2 percent increase in efficacy and potency.

That is a non-answer. The 180 days refers to the base raw material, not the compounded drugs. Specifying the supplier of the raw material means nothing, and they cannot speak to what other suppliers do. Bottom line is they have no stability data. Many years ago another member asked Thriving Pets if they had stability data and it was claimed they were doing it, then supposedly something happened to destroy the samples. Obviously nothing has been done since then.

Basically the Thriving Pets answer is 'contact Teva and ask them for stability studies' is how I take that response.  Everything else is in that email is just static (not relevant).

Regarding the pergolide- the data we have and require our vendors to provide we don't openly publicize for regulatory and competitive reasons. As a compounding pharmacy we are restricted from publicizing certain information that may be perceived as making a claim to a compounded medication. We are able to share specific chemical batch data directly with your veterinarian.
 
Our current quality processes require all of our active ingredients (pergolide included) to have an analysis of the potency, purity, and impurities (among many other tests). Additionally, all of our vendors are located within the US and Canada with an active FDA registration. 
 
I apologize that I am not able to provide you with specific documentation at this time, but would gladly speak to you further regarding any questions you may have on the sources or quality of the medications we provide.
