Seasonal Rise and Pergolide

Blood was drawn on August 22, 2022 to test ACTH level for Whistler. The test result from IDEXX came back as 8.9 pmol/L (ref. range 2.0 - 10.0 pmol/L). In doing research on ECIR site for acceptable level of ACTH during seasonal rise, I came across differing numbers. For example, one message said ACTH levels should be in the low 20's pg/ml (Cornell) year round whereas another message stated an acceptable level of ACTH during seasonal rise as 49 pg/ml (Cornell)  and up to 29 pg/ml (Cornell) for the rest of the year. What does ECIR recommend as safe levels of ACTH for both Cornell and IDEXX based on their ref. ranges? Also is there a simple method for converting pmol/L to / from pg/ml?

Whistler has been on 3 mg of Pergolide since September 27, 2022. From mid July 2022 he has shown a marked decrease in topline, and, ribs, hips and points of buttock are quite noticeable.. Should I increase his Pergolide now and, if so, by how much and for how long? Looking forward to information and direction.

Hi Reta,

Both of those upper numbers you are referencing are for normal horses.  For PPID horses we want to keep their ACTH level in the upper teens/low 20s (in pg/ml terms) year round.  8.9pmol/L translates to 40.4pg/ml as per Adrenocorticotropic hormone (ACTH) conversion to pmol/L, pg/mL, pg/dL, pg/100mL, pg%, pg/L, ng/L . Online converter from conventional units to SI units | UNITSLAB.COM which means Whistler is due for a dose increase.  Were he mine I'd look at bumping him up a full mg to try to get his level down.
