Strongly Suspect PPID no shedding

My mare is definitely IR but had slightly elevated ACTH level in October at 66.
2022 was our first year of testing anything.
She is currently on 1 mg Pracend.
She pees much less on the pracend and seems to eat less.
She is not currently shedding at all.
My neighbors horses are shedding like crazy.
My vet has said there is no need to do a TSH stim test due to her symptoms responding to the pracend.
Is it advisable to stop the pracend for 3 weeks and do the stim test anyway to get a base line ACTH and a Stim level baseline without prascend?

Hi Lesley,

If you want a confirmed diagnosis, do the Stim test.

The shedding doesn't always resolve with pergolide.

Kirsten, thank you.
I don't really want to take her off the pergolide for 3 weeks for the test because I think it is helping with her insulin somewhat. So just wondering if there is any other reason to do it?
I can take ACTH level again in June and if it stays at 14 would I increase the pergolide dose for the rise or not?

June really isn't close enough to the peak of the rise to tell. I would suggest mid to late August.

Should I take it in June and Aug?
When is it too late to increase the pergolide for the rise if it needs to be raised?

Hi Leslie,

You would test in August to see if her ACTH level is going up or not.  As Dr. Kellon said, no reason to test in June if she's still on pergolide.
