Hi Dr Kellon,

Latest labs:

Insulin 52.54 down from 62
TG 792 up from 732

This is with the increase of beet pulp to over about 4.2 lb per day and 17 % of the WB mixed in.

Same hay amts, same timothy pellets. Decreased TC sr to 1c twice a day only use as taste tempter for minerals. 

Ertugliflozin is ordered ans should be here hopefully soon.  

Feet are going up and down. I have pulled shoes, got him easy care RX that he keeps pulling off.  He seemed reasonably comfortable and then last night he flared something in both fronts hot, major pulses.. I am thinking collections at this point because of the nature of the flares..but it is a little confusing.  

We are not walking more than 5 mins per day as per his comfort levels dictate.

But last night he was very sore when his feet were hot and pulsing. He didnt have boots on for the day because he kept pulling them off and I left him in deeply bedded shavings.  He is biting his heels occasionally and that makes me think collections. I also think that is why he is pulling the boots off in the doorway to his stall.

Let me know your thoughts on the TG.  GGT is very mildly elevated at 35.   WBC still low, fibrinogen mildy elevated at 300.

Here is some video from today.  This is his first time in these clouds with the blue EVA included pad.  He seems to like.them better than the easy care RX. https://youtu.be/cLsbFBElxaY

I have left him in these to see how he does in them standing for a bit.  He is still fussing with his heels in the doorway of his stall occasionally which makes me think something is still going on.  He doesn't normally do this.

Vinnie looks a little better in the Clouds than in the RX’s to me but still hesitant with a head bob. Dr Kellon suggested arnica gel to rub from the heel bulbs up the back to the fetlock. If boots are rubbing, you’ll see shiny areas before there’s  a skin break. I tried the old fix of something like Swat or Vaseline over the heels. I had no luck with either, but you might. Cayuse did some biting at her coronary bands but never mobilized abscesses that we recognized as such.

Hey Cass thank you. The turns were much better in the Clouds, and he seemed more forward, but I am going to wait and see how the EVA pad and standing go for a bit.

Because he is fussing with his heels in the doorway he has some scuffed spots that I don't think are from boot rubs. 

So in Cay's case do you think she had collections reabsorb?

Hi, Nan. I have no way of knowing if Cayuse had abscesses that resolved without my noticing. When she had a subsolar abscess a few years ago, I eventually found a small hole in her sole about two months after she was lame. https://ecir.groups.io/g/CaseHistory/photo/7296/422076

Hi Nancy,

I would suggest reducing his dose by 25% until the ertugliflozin gets there and start him on gamma oryzanol https://www.farmvet.com/Body-Builder?location=&quantity=1&size=72 at the container dose of 1 tablespoon/day. I'm very familiar with this compound as a muscle builder and have been looking at its ability to lower insulin, improve IR, and lower TGs for a while now. We're planning a study soon.

Do you have any Numotizine?

Thanks Dr K. Yes I do have numotizine and I will definitely get some gamma oryzanol and will reduce invokana by 25%.  If  I was giving 225mg that looks like it brings us back to about 170mg.

Hi Nan, and others reading,

Just to add to the reference Cass made to boot rubs, as someone who learned the hard way, never underestimate how sore they can make a horse.They do not even need to look bruised (ask me how I know that)before they do their dirty work.
Maybe taking pictures of a suspected area would help see something that the naked eye missed,in suspicious cases.Don't know.

Thanks Lorna, It is very possible that he is heel sore from fussing in the doorway. He puts his heel on the doorway entrance and pulls back like he is itching. But it is very intentional.  He does this with both heels.   He definitely is telling me something is bothering him there, and the only other time he has done this is when he has collections brewing. He will also bite the side of the heel that the collection is usually on.  It is the medial side on the LF and the lateral side on the RF. 

This was also happening before the boots when I had him wrapped with diapers.  I agree that this could cause discomfort, but my gut is telling me this is more than heel soreness. I think...:)

Nan,my horses did exactly that biting.....using the doorway aside.....when heels/coronary bands were issues. Trying to get rid of discomfort.

I've been wondering whether or not to say this,since viewing a video can be so deceptive,but on both most recent videos something has bothered me about positions of  the labels.On one video the LF label seemed to look off centre. In this last video I thought same of RF. It must be an optical illusion,or just where the boots must be for his feet. But it has nagged me that they might be looser than optimal. Being on the ground may present a totally different picture,so just ignore me. Trying to be a help , not a hindrance.

Hi Lorna,

You are correct that they don't stay centered. Perhaps the shape of the foot rn but he is also pigeon toed which creates a little imbalance. Boots have never stayed center on him. They are snug too which is an effort to get on the foot. He is wearing a 2 on the LF and a 3 on the RF.

Nancy, my gelding is slightly pigeon-toed and boots twist on his high foot. Currently I use Flex boots for him because I can adjust the tension on each side of the hoof differently—so I do up the strap more tightly on the lateral side to prevent the boot twisting inward, so far this works the best for us. You can use a pad in the Flex, but not a thick one. I couldn’t see any recent hoof photos in Vinnie’s album, but the Flex probably won’t work on a very distorted hoof either. Anyway, just a thought. You can also use a thin shim of some sort—felt is good—to fill any gap between the boot and hoof wall, which can sometimes help prevent twisting.

Duh! I should never reply via phone. I suppose you have considered open heeled boots such as Scoot Boots. Taking a heat gun to Easy Boot Gloves to soften them up before putting then on made for a very tight fit.

Good. I'd leave him out of the boots for a while , use the Numotize on soles, heels and coronary bands, diaper or other soft wrap on the feet.

Thanks Dr Kellon, I just finished wrapping with numotizine. I definitely think collections. Rf is worse than LF (rare!!) 

He looked pretty bad today, but he is such a trooper!!!
